Do You Really Know What’s in Your Supplements?

Millions of people take dietary supplements every day in hopes of improving their health. Multivitamins, vitamin D, magnesium, fish oil, vitamin C, calcium, B vitamins and countless other products fill medicine cabinets and kitchen cupboards across America, and for many people, taking them has simply become part of their daily routine.

But how many people have ever stopped to ask: Do I actually know what I’m taking, why I’m taking it, whether I need it and whether what’s printed on the label is actually what I’m getting?

Walk through the supplement aisle of almost any store and it’s easy to understand why people are confused. Bottles promise immune support, heart health, better sleep, increased energy, stronger bones and seemingly everything in between, while one product contains 100% of the Daily Value, another contains 500% and another contains 2,000%, making it easy to assume that if some of a nutrient is good, more must be better.

Vitamins and minerals are biologically active substances that your body needs in specific amounts. Too little can create problems, while more than you need may provide no additional benefit and, in some cases, can create problems of its own. Certain supplements can also interact with medications or interfere with the absorption of other nutrients and medications.

This is why I don’t believe the right question is, “What supplements should everybody take?” A better approach is to determine what your body actually needs, what you’re already getting from food, whether there is evidence that supplementation would benefit you and, if so, the appropriate amount, form and quality of the product you’re considering.

Throughout this article, we’re going to take a closer look at seven widely used and commonly discussed dietary supplements, what the science tells us about them, when supplementation may make sense, what to look for when choosing a quality product and what supplements can and cannot do for your health.

Multivitamins and Minerals

Multivitamins are among the most commonly used dietary supplements and are often taken to help cover nutrients that may be missing from the diet. They can be useful when someone’s diet consistently falls short of certain vitamins or minerals because of limited food variety, dietary restrictions, increased nutritional requirements or other circumstances that make meeting those needs through food more difficult.

What a multivitamin cannot do is turn a poor diet into a healthy one or guarantee protection against chronic disease. A large NIH-led analysis involving nearly 400,000 generally healthy U.S. adults followed for more than 20 years found that regular multivitamin use was not associated with a lower risk of death.

That doesn’t mean multivitamins have no value, it means their value needs to be understood in the proper context. Their purpose is to help provide nutrients that may otherwise be inadequate, not compensate for poor nutrition or protect someone from chronic disease simply because they take a multivitamin every day.

What I Look for in a Multivitamin

I generally look for a reasonably dosed product rather than one containing enormous percentages of the Daily Value simply because larger numbers look more impressive on the label. The amount should make sense in the context of what someone is already getting through food.

I also want transparent labeling that clearly identifies each nutrient and the amount provided. Whenever possible, I prefer meaningful independent quality verification from organizations such as USP or NSF, which can provide additional assurance that a product has been independently evaluated against specified standards for areas such as ingredient identity, manufacturing quality and contaminants.

Third-party verification doesn’t tell you that you need the supplement or that taking it will improve your health. It tells you something about the quality and integrity of the product you’re buying.

The goal isn’t to find the multivitamin with the longest ingredient list or the biggest numbers on the label, it’s to choose a quality product that provides appropriate amounts of the nutrients you actually need.

Vitamin D3

Vitamin D plays an important role in calcium absorption and bone health, while also contributing to normal muscle, nerve and immune function. The body can produce vitamin D in the skin following exposure to sunlight, which means vitamin D status can be influenced by sun exposure, geographic location, season, age and skin pigmentation, in addition to dietary intake.

Someone who is deficient has a very different nutritional need from someone whose vitamin D status is already adequate, which is why supplementation shouldn’t automatically be approached as one-size-fits-all.

When there is reason to question vitamin D status, a blood test measuring 25-hydroxyvitamin D can provide useful information when determining whether supplementation may be appropriate. This becomes particularly important when considering higher doses because vitamin D is fat-soluble, and excessive intake from supplements over time can cause toxicity, including abnormally high calcium levels.

What I Look for in a Vitamin D Supplement

When supplementation is appropriate, vitamin D3, or cholecalciferol, is a commonly used form. I look for a clearly stated dose, reputable manufacturing and meaningful independent quality verification whenever possible.

The amount should make sense for the individual based on factors such as dietary intake, sun exposure, current vitamin D status and the reason for supplementation, rather than automatically reaching for the highest-dose product on the shelf.

Omega-3 EPA and DHA

Omega-3 supplements, commonly sold as fish oil, are a good example of why it’s important to understand what a product actually contains rather than relying on the marketing on the front of the bottle.

EPA and DHA are long-chain omega-3 fatty acids found primarily in fatty fish and seafood. They have been extensively studied in cardiovascular health, and supplemental EPA and DHA can lower elevated triglyceride levels.

That doesn’t mean everyone needs a fish oil supplement. Fatty fish provides EPA and DHA along with high-quality protein, vitamins, minerals and other nutrients, which is why I prefer looking at someone’s diet before automatically recommending supplementation.

Supplementation may be useful for people who consume little or no fatty fish, or when EPA and DHA are being used for a specific clinical reason under the guidance of a healthcare professional.

What I Look for in an Omega-3 Supplement

One of the biggest mistakes people make when comparing fish oil supplements is focusing on the large number printed on the front of the bottle. A product labeled “1,000 mg fish oil” does not necessarily provide 1,000 mg of EPA and DHA, so I look at the Supplement Facts panel to determine how much EPA and DHA the product actually provides per serving.

Quality and storage also matter because the oils in fish oil supplements are susceptible to oxidation. I look for transparent labeling, reputable manufacturing, appropriate storage recommendations and meaningful independent quality testing whenever possible.

What matters isn’t how impressive the number on the front of the bottle looks, but what the product actually provides and whether that amount makes sense for the reason you’re taking it.

Magnesium

Magnesium has become one of the most talked-about supplements, with claims ranging from improving sleep and reducing muscle cramps to helping with anxiety and metabolic health. While some of those claims extend beyond what the evidence can consistently support, magnesium is an essential mineral involved in hundreds of enzymatic reactions throughout the body, including normal muscle and nerve function, blood glucose and blood pressure regulation, energy production and bone health.

The fact that magnesium is essential, however, doesn’t mean everyone needs to supplement it. Foods such as nuts, seeds, legumes, whole grains and leafy green vegetables can provide meaningful amounts, so the first question should be whether someone’s diet is meeting their needs and whether there is a legitimate reason to consider supplementation.

The form also matters because different magnesium compounds vary in how well they are absorbed and how they affect the gastrointestinal system. Magnesium citrate and chloride tend to be better absorbed than magnesium oxide, while some forms are more likely to produce a laxative effect.

What I Look for in a Magnesium Supplement

One of the most important numbers on a magnesium supplement is the amount of elemental magnesium listed on the Supplement Facts panel. That’s the amount of magnesium the product is actually providing, rather than the total weight of the magnesium compound used to deliver it.

I also want to know which form is being used and whether it makes sense for the reason someone is taking it, rather than choosing whichever bottle has the largest number or strongest marketing claims.

Supplemental magnesium can cause diarrhea, nausea and abdominal cramping, particularly at higher amounts, and it can interact with certain medications, making both the amount and the individual’s circumstances important considerations when supplementing.

Vitamin C

Vitamin C is an essential nutrient involved in several important functions throughout the body, including collagen synthesis, immune function and antioxidant activity, while also improving the absorption of nonheme iron found in plant foods.

For most people eating a varied diet that regularly includes fruits and vegetables, meaningful amounts of vitamin C can be obtained through food. Because vitamin C is commonly associated with immune health, however, it’s easy to assume that increasingly larger supplemental doses must provide increasingly greater benefits.

Meeting your body’s nutritional requirement and consuming amounts far beyond that requirement are two different things, and taking more vitamin C does not mean your body continues receiving proportionally greater benefit.

What I Look for in a Vitamin C Supplement

Before considering supplemental vitamin C, I want to know whether someone’s diet is already providing adequate amounts and whether there is a legitimate reason for adding more. If supplementation is appropriate, I generally see little reason to chase enormous doses or expensive formulations without a specific reason for doing so.

The goal is to meet the body’s needs, preferably through food when practical, and supplement when there is a legitimate reason.

Vitamin B12

Vitamin B12 deserves particular attention because there are circumstances where supplementation can be extremely useful. B12 is required for healthy red blood cell formation, neurological function and DNA synthesis, and there are identifiable groups of people who may have difficulty obtaining or absorbing adequate amounts.

Older adults may have reduced absorption, people following strict vegan diets have very limited natural dietary sources of B12, certain gastrointestinal conditions and surgeries can interfere with absorption, and some medications can also affect B12 status.

In these situations, supplementation can address a legitimate nutritional vulnerability, but the reason for using it matters because inadequate dietary intake and impaired absorption are not necessarily the same problem.

What I Look for in a Vitamin B12 Supplement

Before choosing a B12 supplement, I want to understand why it’s needed. Someone whose diet provides very little B12 and someone who has difficulty absorbing it may require different approaches, so the appropriate form, amount and method of supplementation should be based on the individual’s circumstances rather than the claims on the front of the bottle.

Calcium

Calcium is essential for maintaining bone structure and also plays important roles in muscle contraction, nerve transmission and other physiological processes. However, the fact that calcium is essential doesn’t mean everyone automatically benefits from taking a calcium supplement.

Before considering supplementation, I want to know how much calcium someone is already getting from food. Dairy products, fortified foods, certain fish and some vegetables can provide meaningful amounts, and if dietary intake is already adequate, adding supplemental calcium simply because “calcium is good for your bones” may not provide additional benefit.

The objective is to meet the body’s calcium needs, not assume that exceeding them will produce better results.

What I Look for in a Calcium Supplement

If dietary intake isn’t sufficient and supplementation is appropriate, I consider the amount, form and timing of the supplement along with total daily calcium intake from both food and supplements.

Calcium can also interact with certain medications and interfere with the absorption of some nutrients and medications when taken together, so someone’s diet, medications and individual needs should all be considered when determining whether supplementation is appropriate and how much is needed.

What I Look for Before Recommending a Supplement

Before I recommend a supplement, I don’t begin with the brand, I begin with the reason. What are we trying to accomplish, is there evidence supporting its use, can the person’s needs reasonably be met through food, and is there an identified deficiency, inadequate dietary intake, increased nutritional requirement or another legitimate reason for supplementation?

Once we’ve established that there is a reason to supplement, I consider the appropriate form and amount along with medications, other supplements or medical circumstances that could affect its use. Only then do I evaluate the product itself.

I want transparent labeling that tells me exactly what I’m getting and how much. I’m cautious with proprietary blends that make it difficult to determine the amount of each active ingredient, and I have little interest in products containing tiny amounts of impressive-sounding ingredients simply so those ingredients can appear on the label.

I also look for reputable manufacturing and meaningful third-party quality verification when available, but price matters too. Expensive doesn’t automatically mean better, and if a less expensive product provides the same properly dosed ingredient, comparable quality and legitimate independent verification, paying considerably more for packaging and marketing doesn’t make it a better supplement.

The supplement industry is very good at convincing people to start with the product. I believe we should start with the reason. Determine what you’re trying to accomplish and whether supplementation is actually appropriate, then find the product that best meets that need.

Bottom Line

Supplements are tools, and when used appropriately they can help fill nutritional gaps, address specific deficiencies and, in certain circumstances, provide meaningful health benefits. What they cannot do is compensate for an unhealthy lifestyle or replace the foundation that good health is built on.

You cannot out-supplement a diet dominated by ultra-processed food, chronic sleep deprivation or physical inactivity, and you cannot build metabolic health from the supplement aisle. That foundation comes from eating nutrient-dense, minimally processed foods, getting adequate protein, exercising regularly, building and preserving muscle, getting adequate sleep, managing stress and staying hydrated.

Once that foundation is in place, if there is a legitimate nutritional gap or an evidence-based reason to add a supplement, choose the right supplement, in the appropriate form and amount, for the right reason.

Don’t take something simply because everyone else is taking it, because a social media influencer recommended it or because the front of the bottle makes an impressive promise. Understand what you’re taking, why you’re taking it, whether you actually need it and what you realistically expect it to accomplish.

The goal isn’t to take more supplements. The goal is to make better-informed decisions about your health by understanding the WHY behind what you’re doing.
